Rack Card Request Form

This form is for requesting a new Rack Card from the Graphic Artist on Campus. The design is built with a template, So All we need are the information for the fields within the Rack Card. After you submit your form, you can expect a proof of the new Rack Card to be emailed to you within two months.

  • Credentials

    Use the dropdown menus to indicate the number of credentials and their estimated time for completion for your program. If your program does not provide one of the credentials listed below, skip that item.
  • If necessary, provide any additional comments about the credentials that students can pursue with your Program and/or the amount of time required to obtain those credentials in the box below.

  • Please type several careers that your program prepares students for.
  • Please type the median salary of someone who has obtained a degree in this program (This information may be accessed in the most recent Desktop Audit).

    Please provide the following details for your program’s department head and/or lead faculty

  • Photography

    Please provide a preferred day(s)/time(s) and location for a photographer to come by and take pictures of your students/classroom. Try to pick a time that will yield interesting photos (a day in class where you may be doing an interesting activity or project).
